CGL Packaging and Shipping Instructions

Samples are accepted Monday through Friday.

Packaging:

All samples should be appropriately packaged to avoid crushing, breakage or leakage.  Keep products of conception and fetal tissue refrigerated. Keep fresh frozen tumor tissue frozen by shipping on dry ice. Send all other sample types at room temperature and protect from excessive heat.  Test Request Form MUST accompany all samples.

Shipping:

Test kits and return shipping are available, as needed.  Call 206-598-8684 to request kits.

Ship via express carrier (such as FedEx, UPS or DHL)

International Samples:

An import permit is not required by the USDA in order to ship a sample to the CGL from outside the continental United States. However, to ensure that the sample arrives quickly, you may want to contact your international courier for clarification.  Indicating that the enclosed sample is for clinical diagnostic testing and has no commercial value will often expedite shipment.
